Getting Your Parts
You have two easy options available to get the part you need:
Online Form
Our lab preference would be that you simply review and complete our custom online form and a member from our team will receive the request via email.
Fax Us Your Request
Many offices choose to fax the referral surgeon’s letter. We will order the necessary parts you need on your behalf and have them shipped directly to you in one or two business days (based on your preference). This will allow us to get you the parts you need with no markup!
